Hormone Replacement Therapy for Women: An Evidence-Based Guide to Estrogen, Progesterone & Testosterone
For twenty years, a single misread study frightened a generation of women away from one of the most effective therapies in medicine. Here is what the evidence actually says — how each hormone works, what it protects, which myths to retire, and why this is a decision that belongs with a physician, not a headline.
Few areas of medicine have been as badly misunderstood — by patients and physicians alike — as hormone therapy for women. When the Women’s Health Initiative (WHI) reported in 2002, the headlines were catastrophic, prescriptions collapsed almost overnight, and millions of women were left to move through menopause with hot flashes, sleepless nights, thinning bones, and a quiet loss of vitality that they were told simply to endure. Two decades of reanalysis have since told a very different and more hopeful story: that when therapy is started, which hormones are used, and how they are delivered change the risk–benefit picture entirely, and that for most healthy women near menopause, the benefits of well-designed hormone therapy clearly outweigh the risks. This review walks through the three hormones that matter — estrogen, progesterone, and testosterone — explains what each one actually does for symptoms and for long-term health, dismantles the myths that still scare women away, and is equally honest about where the wellness industry now overtreats. Done correctly and guided by a physician, hormone therapy is not merely symptom relief; it is one of the most powerful tools we have for preserving a woman’s bones, brain, sexual health, and quality of life for decades.
The WHI Reset: How One Study Was Misread for Twenty Years
The 2002 WHI trial studied one specific regimen — oral conjugated equine estrogen plus a synthetic progestin (medroxyprogesterone) — in women whose average age was 63, more than a decade past menopause. It reported small increases in breast cancer, stroke, and clots, and the nuance was lost in the panic. What the long follow-up has since made clear is that the picture depends almost entirely on timing and formulation. In the 18-year follow-up of nearly 27,000 women, hormone therapy was associated with no increase in all-cause, cardiovascular, or cancer mortality — it neither shortened nor lengthened life overall — and younger women (50–59) consistently fared better than older ones.
The single most important idea to come out of the reanalysis is the “timing hypothesis,” or window of opportunity: hormone therapy started before age 60 or within 10 years of menopause tends to be protective or neutral for the heart, while starting it many years later, in women with established arterial disease, is where the risk appears. The ELITE trial tested this directly and found that estradiol slowed the progression of atherosclerosis in women who started early after menopause, but not in those who started late. In other words, the WHI didn’t prove hormones are dangerous — it proved that who you treat, and when, matters enormously.

Estrogen: The Foundation
Estrogen is the hormone whose decline drives the classic menopausal picture: hot flashes and night sweats (vasomotor symptoms), disrupted sleep, mood shifts, brain fog, vaginal dryness, painful intercourse, recurrent urinary infections, and accelerating bone loss. It is also the most effective treatment for those symptoms by a wide margin — and in late 2024 the UK’s NICE guideline reaffirmed hormone therapy as first-line for menopausal symptoms, a notable shift back toward treating rather than enduring. The key clinical decision is route of delivery. Because transdermal estradiol (patch, gel, or spray) is absorbed through the skin and bypasses first-pass liver metabolism, it carries a substantially lower risk of blood clots and stroke than oral estrogen — which is why it is the evidence-based default, especially for women with any clot or cardiovascular risk.
Local (vaginal) estrogen: safe, underused, and separate from systemic risk
One of the most important distinctions in all of women’s health is that vaginal estrogen is not the same as systemic estrogen. Low-dose vaginal estradiol (tablet, cream, or ring), or vaginal DHEA (prasterone), treats the dryness, pain, and recurrent urinary tract infections of genitourinary syndrome of menopause with minimal absorption into the bloodstream. It requires no progesterone, carries negligible systemic risk, can be used indefinitely, and — with an oncologist’s input — is often appropriate even for many breast cancer survivors. Countless women suffer silently with these symptoms because they fear “hormones”; local estrogen is one of the safest prescriptions in medicine.
Bone, heart, and the systemic picture
Systemic estrogen is one of the few therapies proven in randomized trials to reduce fractures — including hip and spine — by preserving bone density, and this benefit is a major reason therapy is recommended in women with early menopause or elevated fracture risk. For the heart, the timing hypothesis applies: begun within the window, estrogen appears cardioprotective or neutral. For women with premature ovarian insufficiency or early menopause (before ~45), hormone therapy is not optional optimization but standard of care, generally continued to around the natural age of menopause (~51) to protect the bones, heart, and brain from the consequences of losing estrogen decades too soon.
Progesterone: The Essential Partner
Any woman who still has her uterus and takes systemic estrogen must also take a progestogen. The reason is non-negotiable: unopposed estrogen stimulates the uterine lining and, over time, raises the risk of endometrial hyperplasia and cancer. Adequate progesterone prevents this. Women who have had a hysterectomy generally do not need it. But which progestogen is used is not a trivial detail — it may be one of the most consequential choices in the whole regimen.
Micronized progesterone vs. synthetic progestins
Here lies a crucial and under-appreciated point: much of the breast cancer signal in the original WHI came not from estrogen but from the synthetic progestin (medroxyprogesterone) it used. Oral micronized progesterone — which is body-identical to what the ovaries make — appears to carry a more favorable breast and cardiovascular profile. Observational and systematic-review data suggest it is associated with less breast cancer risk than older synthetic progestins, at least over the first several years of use. It has the added benefit of mild sedation, so taken at bedtime it often improves sleep. For women who don’t tolerate oral progesterone, a levonorgestrel IUD delivers excellent local endometrial protection. This is exactly the kind of choice that separates a modern, evidence-based regimen from a dated one.
Testosterone: The Overlooked Hormone
Testosterone is not just a male hormone — women produce it too, and it falls with age. Its role in women’s health was long ignored, and the pendulum is now swinging, sometimes too far. The honest, evidence-based position is precise: the one indication supported by high-quality randomized trials is hypoactive sexual desire disorder (HSDD) — distressing low sexual desire in postmenopausal women, after other contributors (relationship factors, mood, medications, vaginal dryness) have been addressed. A large systematic review and meta-analysis found that testosterone significantly improved sexual desire, arousal, orgasm, pleasure, and satisfaction while reducing sexual distress. That is a genuine, life-improving benefit for the right woman.
The same evidence base is equally clear about the limits. The Global Consensus Position Statement — endorsed by every major menopause and endocrine society — concludes that the data support testosterone only for HSDD, that there is insufficient evidence to recommend it for energy, mood, cognition, muscle, or bone as standalone goals, and — critically — that it must be dosed to physiologic female concentrations. It explicitly warns against formulations that produce male-range levels. In the United States there is no FDA-approved female testosterone product, so treatment means carefully sub-dosing a male preparation (a transdermal cream or gel) with lab monitoring to keep levels in the female range. Used this way, side effects (acne, unwanted hair) are uncommon and mild; used carelessly, it causes exactly the virilizing effects women fear.
The Longevity Case: Bone, Heart & Brain
Beyond symptom relief, estrogen’s decline accelerates several of the diseases that most shape how women age. Bone is the clearest win: estrogen reliably preserves bone density and reduces osteoporotic fractures, a benefit with genuine longevity implications, since a hip fracture in an older woman is a life-altering — sometimes life-ending — event. For the heart, the timing hypothesis holds: therapy begun in the window is associated with favorable or neutral cardiovascular effects, though hormones are not prescribed for the sole purpose of preventing heart disease. For the brain, the evidence is genuinely nuanced — early-window therapy appears cognitively neutral to possibly favorable, while starting hormones for the first time after 65 has been linked to higher dementia risk, another reason timing is everything.
The intellectually honest framing, echoed by the major societies, is this: hormone therapy is not a general-purpose anti-aging or chronic-disease-prevention drug for every woman, and it should not be started in a 70-year-old to prevent dementia or a heart attack. But for a symptomatic woman near menopause, the same therapy that relieves her hot flashes is simultaneously protecting her bones and, on balance, doing her cardiovascular and cognitive health no harm and possibly some good. Preserving function before it is lost — bone before fracture, sexual health before it erodes, sleep and mood before they unravel — is the essence of preventive medicine, and it is where hormone therapy, used in the right woman at the right time, earns its place.
Myths vs. Evidence
The Long View
A generation under-treated, an industry over-correcting, and the case for a guide
For twenty years, menopausal women were failed by an excess of caution. Physicians, spooked by the WHI headlines and undertrained in menopause, too often offered nothing but an antidepressant and a shrug. That vacuum was real, and it did real harm — sleepless years, fractured hips, lost libido, careers derailed by symptoms dismissed as inevitable. The correction was overdue. But vacuums attract more than good medicine, and the same cultural moment that finally took women’s hormones seriously has also birthed a booming, largely unregulated “hormone optimization” industry — compounding pharmacies, pellet clinics, and subscription telehealth — that now risks over-correcting into a different kind of harm.
The clearest example is the hormone pellet. Marketed as “natural” and “bioidentical,” compounded estradiol and testosterone pellets are not FDA-approved, cannot be removed or dose-adjusted once implanted, and repeatedly drive hormone levels well above the physiologic range — producing the acne, hair loss, and abnormal labs that careful therapy is designed to avoid. The irony is sharp: the truly body-identical hormones these clinics invoke are available as regulated, tested, FDA-approved products, delivered transdermally and orally with far better safety data. “Bioidentical” was always a marketing word, not a safety category. The same is true of testosterone dosed to male levels in women, and of hormones sold to asymptomatic women as an anti-aging cure-all the evidence does not support.
That is the entire argument for an evidence-based physician, and it is not a marketing line — it is a function. A physician confirms you are in the window where benefits outweigh risks, and screens for the contraindications — a history of estrogen-sensitive cancer, unexplained bleeding, prior clot or stroke — that change the calculus. A physician chooses transdermal estradiol over oral to lower your clot risk, micronized progesterone over an older synthetic to protect both your uterus and your breast, and physiologic testosterone only if you have a real indication for it. A physician monitors, adjusts, and revisits the decision every year instead of implanting a pellet and moving on. Used this way, hormone therapy is genuinely restorative — and preserving a woman’s bones, brain, sexual health, and vitality through the second half of her life is preventive medicine in the most literal sense.
So where does that leave a thoughtful woman? Not frightened, and not credulous. Hormone therapy is legitimately effective and, for the right patient at the right time, legitimately worth having; the science supporting it is stronger and more reassuring than the fear that displaced it. The difference between a result and a regret is almost always whether someone with medical judgment — not a headline, and not an algorithm optimizing for a pellet subscription — decided whether you needed it, which hormones to use, how to deliver them, and what to watch. That is the whole project: not the average woman in a scare or a sales pitch, but the actual one in the room.
